The entry form is now available on the home page. Simply click on the 2003 Entry Form and Rules and you can print out the form. After printing the form you can either mail it to:

Early bird entries must be submitted prior to January 24th. Single tournament entries may be submitted beginning January 31, 2003 and as long as space allows up to 2 weeks prior to the tournament date.
